$195 Million in Grants Available for Statewide Health Care Facility Transformation Program


Title:  Statewide Health Care Facility Transformation Program (SHCFTP)

Provides grants to health care providers that primarily support capital projects—but may include non-capital expenses such as debt restructuring – for the purpose of strengthening and protecting continued access to health care services in communities throughout New York State and are associated with a merger, consolidation, or significant corporate restructuring activity that is part of an overall transformation plan intended to create a financially sustainable system of care.
NOTE:  In addition to other evaluation criteria (RFA, p. 7), applicants must provide significant data (through a community needs assessment) on how the project supports the identified needs of residents in the community or communities, preserves essential health services in the community, and the extent to which the applicant has engaged the community affected to shape the eligible project.

Eligible Applicants:  
Provider organizations including:

  • General Hospitals licensed under Article 28 of PHL
  • Residential Health Care Facilities licensed under Article 28 of PHL
  • Community-based health care providers which are defined for the purposes of this RFA as:
    • Diagnostic and Treatment Centers Certified under Article 28 of PHL
    • Mental Health Clinics certified or licensed under Article 31 of the MHL
    • Alcohol and Substance Abuse Treatment Clinics certified or licensed under Article 32 of the MHL.
    • Primary Care providers
    • Home Care providers certified or licensed under Article 36 of the PHL

Funding Available:  Up to $195 million is available to support this RFA for contract periods of 60 months. A minimum of $30 million will be made available for community-based health care provider projects. Projects shall NOT receive an award under both this RFA and the Kings County Health Care Transformation Program or the Oneida County Health Care Transformation Program (PHL 2825-a or b respectively). Awards made under this RFA are determined on a non-competitive basis and are awarded at the discretion of the Commissioner of Health for purposes described in the RFA (based on review by teams at DOH and DASNY).

For-profit applicants have a 30% goal (15% minority; 15% women-owned) to use Minority and Women-Owned Business Enterprises (MBE/WBE) for this solicitation based on the total value of their budget.
